Under Section 2799B-6 of the Public Health Service Act, health care providers are required to provide a good faith estimate of expected charges for items and services to individuals who are not enrolled in a plan or coverage or a Federal health care program, or not seeking to file a claim with their plan or coverage both orally and in writing, upon request at the time of scheduling health care items and services.
RATES:
I offer a free 10-minute phone consultation, so you can get a sense of whether we have a good match. It is also an opportunity for me to decide whether or not I will be able to help you.
$150 for a 53-minute individual session
$175 for a 60-minute session
CANCELLATION POLICY:
If you do not show up for your scheduled therapy appointment, and you have not notified me at least 48 hours in advance, you will be required to pay $150 for that time.
INSURANCE:
At this time I accept United Health Care. Please check with your Insurance plan for Mental Health benefit information. I do offer a “super bill” for your insurance for out of network benefits.
PAYMENT:
Cash, credit card or FSA cards are accepted for payment at the time of service. A credit card is required to hold your appointment spot.
TELE-THERAPY
I offer tele-therapy for Texas, Minnesota, Ohio, and New York Please consult your insurance company for tele-therapy benefit coverage.
